PROGRAM EVALUATION FORM
All student organizations that received funding from the University or Cultural Fund are required to submit Program Evaluation Forms for each funded program. Evaluations are due within TWO WEEKS after the funded program. Eligibility for the next funding cycle is contingent on the submission of these evaluations. Additional questions? Email vcw @ duke.edu.
